risk of injury involved in training and participating in any sport. I understand that Olympium Rhythmic
Gymnastics Club and Gymnastics Ontario have tried to create a safe and controlled
environment for participation and that the Club has established rules for
participation on and about the gymnastics area that must be followed. I understand that failure to comply with any
of the policies and rules of the Club and/or Gymnastics Ontario may result in the
suspension or termination of membership.
I waive the rights of the participant and family named above, to damages
or other costs in the event injury is caused due to participation in gymnastics
or other involvement with the Club/Federation.

RECREATION PROGRAMS

REGISTRATION INFORMATION:
Club Membership Fee - An additional fee of
$85.00 applies to all new club members.
(If your child already has a club uniform/leotard, your club membership
fee will only be $40.00). This fee includes the mandatory Gymnastics Ontario
Membership and a club leotard. Club
membership fees are paid once per gymnastics season.
If you paid GO membership during our
2007 August Summer Camp, disregard the $40.00 fee.
Payment is by cash, cheque, Visa or MasterCard. Cheques are payable to
“Olympium RGC”. Registration MUST be
accompanied by payment.
There is a $50.00 fee for all returned cheques.
Registration - You may
register in person, by mail, or fax. Payment must be included with
completed and signed form, therefore if registering electronically; you MUST
pay by credit card. Space is limited
in each class. Registrations are on a first
come first served basis after the current member’s re-registration period.
Discount - There is a second child discount
(20% of lowest fee) for 2 registrations in any one session within the same
family.
MISCELLANEOUS
INFORMATION:
·
Olympium RGC reserves the right to cancel classes or
request gymnasts to change their selection if there is insufficient enrolment
in a class.
·
Classes may be cancelled and/or rescheduled due to
weather conditions or unforeseen circumstances.
·
A pro-rated refund, less a $20.00 administration fee
is available upon request for withdrawals from a program when the request is
made before the fifth class.
·
The club fee ($85.00 or $45.00) is non-refundable
except in the case where the withdrawal is made before the first class.
·
Parents are requested to remove themselves from the
gymnasium or class area for the duration of the class.
